Hello, thanks for sharing your report details on HCM.
I will highlight the abnormal findings on the urine analysis-
- Cloudy urine: whitish discoloration.
- Protein: positive. Normally it must be absent.
- White Blood cells
- Bacteria: significant

Summarizing these findings it seems that you have a urinary tract infection.
I would suggest a culture & sensitivity report of the urine, to see which specific bacteria is causing it, and which antibiotics the bacteria are sensitive/resistant to, so that appropriate medication can be started.
Also check your blood sugar levels.
A ultrasound of the kidney, ureter bladder and prostate(for males) may also be done in the workup for urine infection.

You have not mentioned any details about your medical history,so I would suggest that your doctor see this report as well.
You should continue to drink good amount of fluids and avoid hold up of urine.
Hope this is helpful.
Take care.
